Youth & GL Registration 2024

ASP volunteers (both youth and adult) need to fill out this form entirely.

To complete registration, you will need these things:

Volunteer's social security number (for CORI background check)
Image (photo or scan) of signed Volunteer Covenant & Release form. Must be signed by volunteer and parent if under 18.
Photo of volunteer's health insurance card, front and back (in case of emergency). Crop images to only show the medical card.
Profile picture, preferrably a square cropped view of volunteer's face.
For group leaders: photo of driver's license (needed for ID and van rental). Crop image to only show the license.

All items are REQUIRED for registration and must be uploaded on next page. Incomplete registrations will be put on wait list. Volunteers (youth and adults) registering for the program will be opted in for text alerts (we use these sparingly). Parents may opt-in on this form or via the web site.

If you already have an account on the portal it would be faster to login to use details from your prior registration.

Prep Retreat (May 10 to 12) *
